The National End of Life Care Programme, works with health and social care services across all sectors in England to improve end of life care for adults by implementing the Department of Health′s End of Life Care Strategy. Their website is designed to support health and social care staff working, in any capacity, with people nearing the end of life.

National End of Life Programme Holistic Common Assessment Document (2010)

Guidance for completing a holistic assessment in patients requiring end of life care, outlining the physical, psychological, social and spiritual assessment domains. This document also highlights the range of existing assessment and planning tools, guidance and relevant policy, and signposts to other resources where appropriate.

Advance Care Planning Guide (2011)

Advance Care Planning (ACP) is process of discussion to help an individual discuss their condition and, if they wish, set on record their choices and preferences regarding their about their care and future care. An example of Advance Care Planning, in the form of a patient held document that provides an opportunity to discuss end of life issues with patients, and explicitly record their wishes. It can form the basis of care planning in multi-disciplinary teams and other services, by facilitating patient choice and minimising inappropriate admissions and interventions.

Liverpool Care Pathway for the Dying Patient Website and Supporting Documents (2009)

The Liverpool Care Pathway (LCP) is an evidence-based framework that is used at the bedside to support those delivering care to the dying patient and their relatives in the last days and hours of life. It transfers the best quality for end of life care of the dying from the hospice movement into other clinical areas, including hospitals, care homes, individuals own homes and hospices.

General Medical Council Good Medical Practice Guidance (2010)

The GMC′s Good Medical Practice Guidance, Treatment and care towards the end of life: good practice in decision making′ sets out the ethical principles underpinning good end of life care, and provides advice to support doctors in approaching these often complex and difficult situations. The GMC website contains the additional materials to support the guidance, including e-learning, case studies and videos.

Dying Matters

Dying Matters is a broad based and inclusive national coalition which aims to change public knowledge, attitudes and behaviours towards death, dying and bereavement. The Dying Matters website contains many resources for the public and professionals, including a series of leaflets that provide support and advice on how to start open and honest conversations about dying.

Cumbria and Lancashire End of Life Care Network

In 2008, the Lancashire and South Cumbria Cancer Network established a new group, the Cumbria and Lancashire End of Life Network, to support the implementation of the National and North West End of Life Care Strategy. Their website contains a wide range of information around end of life care for patients, a relatives, carers, health or social care professionals and those just interested in learning more about end of life care and planning for the future.

Map of Medicine and End of Life Care Pathway in Adults

The Map of Medicine defines care pathway maps that are used to design, implement, communicate and monitor healthcare cross-boundary healthcare services within the NHS. The End of life care in adults pathway, which replaced the previous Palliative care pathway, is a comprehensive pan-specialty broad pathway, that addresses the management of physical symptoms, practical and social needs, psychological and spiritual requirements, support for families and patient′s priorities and preferences. The pathway aims to help ensure all those approaching the end of life have access to appropriate services and receive consistent, high-quality care in all settings, through co-ordinating care and facilitating cross-specialty communications.
